Job summary

Transitions Healthcare North Huntingdon is seeking a Nurse Assessment Coordinator to work with the MDS Coordinator on planning and implementing MDS completion in accordance with facility procedures and state/federal regulations.

The role may be filled as LPNAC or RNAC, requiring licensure in Pennsylvania, CPR/AED certification, and at least one year of nursing experience. Proficiency in MS Office and Outlook is expected, with attention to data accuracy and team collaboration.

Qualifications

• Current licensure as either a Practical Nurse (LPN) or Registered Nurse (RN) in Pennsylvania.
• CPR/AED certification is required.
• Minimum of one year nursing experience.
• Proficiency in MS Office Suite and Outlook.

Responsibilities

• Gathers necessary data for completion of the MDS.
• Assists in identifying significant changes in residents’ condition.
• Monitors resident care plans for accuracy and completion.
• Attends the Medicare meeting.
• Assists in education of the MDS process with nursing staff and the interdisciplinary care team.
• Data entry of MDS information.
• Reviews messages from contracted pharmacy regarding medications needing prior authorization.
