Job Overview

As an MDS Coordinator / Nurse Assessment Coordinator, you will complete and ensure the accuracy of Minimum Data Set (MDS) assessments for all residents. The role involves contributing to personalized resident care plans and ensuring proper clinical reimbursement for services provided.

Key Responsibilities

• Determine Patient Driven Payment Method (PDPM) and expenses associated with potential admissions
• Assess nursing needs of prospective residents and determine appropriate reimbursement levels
• Complete and verify the accuracy of MDS assessments for all residents
• Monitor Case Mix Index (CMI) scores for potential risks or changes affecting reimbursement
• Maintain revenue integrity and compliance with all regulations regarding MDS coding and completion
• Collaborate with interdisciplinary teams for accurate data collection
• Provide education and insights to facility staff and leadership

Qualifications

• Valid RN nursing license in the state
• Preferred: advanced degree or certification
• Experience in long-term care, MDS coordination, or related fields preferred
• Knowledge of regulations governing MDS, EMR, PDP, Medicaid, and Medicare
• Interest and ability to work with aged and chronically ill populations
• Detail-oriented, organized, analytical, and able to work independently
• Excellent communication and interpersonal skills
• Ability to influence and work effectively in a team environment
