Riverside Rehab Center 2 is seeking a dedicated MDS - Nurse Assessment Coordinator (RN) to join our team. Position: MDS Coordinator / RN (Full Time)
What You'll Do
  • Complete and ensure the accuracy of Minimum Data Set (MDS) assessments for residents
  • Contribute to personalized care plans and ensure proper clinical reimbursement
  • Determine Patient Driven Payment Method (PDPM) and assess expenses related to admissions
  • Participate in admitting prospective residents by evaluating nursing needs
  • Monitor Case Mix Index (CMI) scores for potential risks affecting reimbursement
  • Ensure compliance with all state and federal regulations regarding MDS coding and completion
  • Collaborate with interdisciplinary teams for accurate data collection
  • Provide education and insights to facility staff and leadership

Qualifications
  • Valid state RN nursing license
  • Preferred: Advanced degree or certification
  • Experience in long-term care, MDS coordination, or clinical reimbursement is preferred
  • Knowledge of regulations governing MDS, EMR, Medicaid, Medicare, and related requirements is helpful
  • Strong organizational, analytical, and decision-making skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively within a multidisciplinary team
