Job summary

Avir at Victoria is seeking a full-time MDS Nurse to conduct comprehensive assessments of residents, including MDS, and to develop individualized care plans. You will collaborate with an interdisciplinary team to ensure effective care and positive resident experiences.

You will coordinate with healthcare professionals, monitor progress, and communicate with residents and families to address needs and concerns, contributing to high-quality care and outcomes.

Responsibilities

• Conduct comprehensive assessments on residents, including the Minimum Data Set (MDS) and other relevant assessments
• Develop and implement individualized care plans that address the unique needs and goals of each resident
• Coordinate with the interdisciplinary team to ensure that care plans are implemented and updated as needed
• Monitor resident progress and make adjustments to care plans as necessary
• Collaborate with other healthcare professionals to ensure that high-quality care is provided
• Communicate effectively with residents, families, and other stakeholders to ensure that their needs are met and concerns are addressed
