Job summary

Avir at Victoria in Victoria, TX is seeking a full-time MDS Nurse to conduct comprehensive assessments of residents (MDS and other assessments) and develop individualized care plans.

You will work with an interdisciplinary team to implement and adjust care plans, monitor resident progress, and communicate with residents, families, and stakeholders to ensure high-quality care.

Responsibilities

• Conduct comprehensive assessments on residents, including the Minimum Data Set (MDS) and other relevant assessments
• Develop and implement individualized care plans that address the unique needs and goals of each resident
• Coordinate with the interdisciplinary team to ensure that care plans are implemented and updated as needed
• Monitor resident progress and make adjustments to care plans as necessary
• Collaborate with other healthcare professionals to ensure that high-quality care is provided
• Communicate effectively with residents, families, and other stakeholders to ensure that their needs are met and concerns are addressed
