Qualifications

• MDS experience in skilled nursing or long-term care.
• Understanding of CMS regulations related to MDS
• Familiarity with PDPM reimbursement structures
• Strong clinical documentation, auditing and organizational skills
• Proficiency with Electronic Health Record (EHR) systems
• Ability to work independently and in interdisciplinary teams

Responsabilités

• Coordinate, complete, and transmit MDS assessments within regulatory timelines.
• Review clinical records and therapies documentation for data accuracy.
• Participate in interdisciplinary care planning to implement personalized resident plans.
• Monitor Quality Measures and identify opportunities for improvement.
• Collaborate with Director of Nursing and MDS Coordinator for state surveys and audits.
• Guide floor staff on precise daily documentation practices
