Sweetwater Care is seeking a part-time MDS Coordinator in Denver to ensure accurate MDS assessments and care planning. You will coordinate the MDS process from start to finish, collaborate with the care team, and maintain PPS/OBRA schedules to ensure timely reimbursements and compliance.

Ideal candidates hold an active RN or LPN/LVN license, have experience with MDS/RAI processes, and demonstrate meticulous attention to detail.

Responsabilidades

• Completing and coordinating the MDS assessment process from start to finish.
• Ensuring timely and accurate completion of MDS assessments, care plans, and CAAs per RAI guidelines.
• Tracking PPS and OBRA schedules to prevent delays.
• Collaborating with nursing, therapy, social services, and dietary teams to build a complete resident picture.
• Reviewing documentation to support appropriate reimbursement and regulatory compliance.
• Serving as a go-to resource for resident assessment and care planning questions.

What You'll Be Doing

• Completing and coordinating the MDS (Minimum Data Set) assessment process from start to finish

• Ensuring timely and accurate completion of MDS assessments, care plans, and CAAs in line with RAI guidelines

• Tracking PPS and OBRA schedules so nothing slips through the cracks

• Collaborating with nursing, therapy, social services, and dietary teams to capture a complete, accurate picture of each resident

• Reviewing documentation to support appropriate reimbursement and regulatory compliance

• Serving as a go‑to resource for questions about resident assessment and care planning

What You Bring

• Current RN or LPN/LVN license in good standing

• Experience with MDS assessments and the RAI process (skilled nursing facility experience preferred)

• Strong attention to detail — you catch what others miss

• Solid understanding of Medicare/Medicaid regulations and reimbursement processes

• Great communication skills and the ability to work cross‑functionally with a care team

• Comfort with EHR/MDS software systems
