Job summary

Veritas Management Group is seeking a FT LVN MDS Coordinator to lead the development and completion of MDS, CAAs, and Care Plans in line with State and Federal requirements.

You will join the IDT to manage MDS scheduling, ARDs, and ensure accurate documentation, while guiding CNA staff on ADL detailing and MDS item completion.

Qualifications

• LVN licensed nurse with state certification.
• Experience with RAI/MDS processes.
• Two years in skilled or long-term care setting.
• Experience with MDS certification preferred.

Responsibilities

• Coordinate the interdisciplinary team in timely completion of assessments.
• Plan MDS ARDs using MDS Scheduler.
• Communicate ARD selection to IDT.
• Complete the MDS sections as assigned.
• Complete CAAs as required.
• Complete Care Plan items and attend Care Plan meetings.
• Monitor documentation to validate MDS coding.
• Provide teaching and training for MDS item completion to IDT.
• Mentor CNAs and staff on Nursing ADL documentation.

Job Description

Purpose of Job Position: The primary purpose of this position is to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Are Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. The MDS Coordinator participates as part of the facility Interdisciplinary Team (IDT) in the systems and processes to manage patients receiving skilled services as assigned. Performs other nursing duties as assigned.

Essential Functions

• Coordinates the interdisciplinary team in timely completion of the assessments
• Utilizes MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs)
• Communicates ARD selection for specific MDSs to IDT
• Completes the MDS sections as assigned
• Completes CAAs as required
• Completes Care Plan items and attends Care Plan meetings as assigned
• Monitors overall the documentation in the medical record to validate that it supports MDS coding
• Participates in skilled utilization and State Case Mix management as assigned
• Provides teaching and training for MDS item completion to interdisciplinary team members that have responsibility for MDS item completion
• Provides teaching and mentoring of C.N.A. staff along with Director of Nurses and Staff Development related to completion of the Nursing ADL documentation

Education Requirements

LVN
