Job summary

The Center at Zaragoza in El Paso, TX seeks an FT LVN MDS Coordinator to lead the development and completion of MDS, CAAs, and Care Plans in compliance with State and Federal requirements. You will work within the facility’s Interdisciplinary Team to coordinate ARDs and ensure accurate documentation across the medical record.

Responsibilities include teaching CNAs and staff on MDS item completion, coordinating the IDT for timely assessments, and supporting skilled utilization and case-mix

Qualifications

• Licensed Vocational Nurse (LVN) with active license.
• Minimum two years of experience in skilled or long-term care settings.
• Experience with the RAI/MDS process and certification is preferred.

Responsibilities

• Coordinate the interdisciplinary team to ensure timely completion of MDS and related assessments.
• Utilize MDS scheduling tools to plan ARDs and communicate ARD selections to IDT.
• Complete MDS sections, CAAs, and Care Plans as assigned; monitor documentation to support MDS coding.
• Attend Care Plan meetings and provide teaching on MDS item completion to CNAs and other staff.

Looking for a FT LVN MDS to join our Team.Job descriptionPURPOSE OF JOB POSITION:The primary purpose of this position is to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Are Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. The MDS Coordinator participates as part of the facility Interdisciplinary Team (IDT) in the systems and processes to manage patients receiving skilled services as assigned.
