Job summary

Isles of Boynton Nursing and Rehabilitation Center in Boynton Beach, FL is seeking an experienced MDS Director (RN/LPN) to lead our MDS program, ensure accurate documentation, and support reimbursement and regulatory compliance. This leadership role collaborates with inter-disciplinary teams to optimize resident care and outcomes.

The ideal candidate has Florida RN or LPN licensure, 2+ years in MDS coordination/director roles in SNF, and strong knowledge of MDS 3.0, PDPM, and Medicare

Qualifications

• Active Florida RN or LPN license.
• Minimum 2 years MDS Coordinator or MDS Director experience in a Skilled Nursing Facility (SNF).
• Strong knowledge of MDS 3.0, PDPM, Medicare regulations, reimbursement systems, and care planning processes.
• Excellent leadership, organizational, analytical, and communication skills.
• Ability to manage multiple deadlines and maintain compliance in a fast-paced healthcare environment.

Responsibilities

• Oversee accuracy, completeness, and timeliness of all MDS assessments and documentation.
• Ensure compliance with state and federal regulations including PDPM requirements.
• Conduct comprehensive resident assessments and interviews.
• Develop, implement, and monitor individualized care plans for short‑ and long‑term residents.
• Lead and participate in interdisciplinary care plan meetings.
• Collaborate with nursing staff, physicians, therapists, residents, and families to improve outcomes.
• Provide leadership, guidance, and ongoing education to nursing staff on MDS processes.
• Conduct in-service training related to MDS 3.0, RAPS, CAAs, PDPM, and Medicare guidelines.
• Maintain and manage MDS assessment schedules and care plan timelines.
• Monitor reimbursement opportunities while ensuring regulatory compliance and clinical integrity.

Position: MDS Director (RN/LPN)
Facility: Isles of Boynton Nursing and Rehabilitation Center
Location: Boynton Beach, FL
Status: Full-Time | 8:00 AM - 4:30 PM
Compensation: $46-$49 hourly (based on experience)

Position Summary

Isles of Boynton Nursing and Rehabilitation Center is seeking an experienced and highly organized MDS Director (RN/LPN) to lead our MDS program and ensure accuracy, compliance, and excellence in resident-centered care. This leadership role is essential in supporting appropriate clinical documentation, reimbursement optimization, regulatory compliance, and interdisciplinary collaboration.

Key Responsibilities

• Oversee the accuracy, completeness, and timeliness of all MDS assessments and supporting documentation.
• Ensure compliance with state and federal regulations, including Medicare, Medicaid, and PDPM requirements.
• Conduct comprehensive resident assessments and interviews.
• Develop, implement, and monitor individualized care plans for short-term and long-term residents.
• Lead and participate in interdisciplinary care plan meetings.
• Collaborate with nursing staff, physicians, therapists, specialists, residents, and families to improve clinical outcomes.

• Conduct in-service training related to MDS 3.0, RAPS, CAAs, PDPM, and Medicare guidelines.
• Maintain and manage MDS assessment schedules and care plan timelines for all residents.
• Monitor reimbursement opportunities while ensuring regulatory compliance and clinical integrity.
• Support quality assurance and performance improvement initiatives related to clinical documentation and resident outcomes.

Qualifications

Required

• Active, unencumbered Florida RN or LPN license.
• Minimum of two (2) years of MDS Coordinator or MDS Director experience in a Skilled Nursing Facility (SNF).
• Strong knowledge of MDS 3.0, PDPM, Medicare regulations, reimbursement systems, and care planning processes.
• Excellent leadership, organizational, analytical, and communication skills.
• Ability to manage multiple deadlines and maintain compliance in a fast-paced healthcare environment.

Preferred

• RAC-CT certification.
• Experience with electronic medical record (EMR) systems utilized in long-term care settings.
• Prior supervisory or leadership experience.
