Qualifications

• Active Florida RN or LPN license in good standing.
• Minimum of 3 years’ experience as an MDS Coordinator in a Skilled Nursing Facility.
• Strong working knowledge of MDS, PDPM, Medicare guidelines, and care-planning processes.
• Excellent organizational, leadership, and communication skills.

Responsibilities

• Ensure the delivery of high-quality, compliant care for long-term care residents.
• Monitor accuracy and timeliness of MDS assessments and documentation.
• Conduct comprehensive initial assessments and resident interviews.
• Develop and oversee individualized care plans with both short-term and long-term goals.
• Lead and participate in interdisciplinary team meetings.
• Collaborate with caregivers, families, and clinical specialists to optimize resident outcomes.
• Provide leadership and guidance to nursing staff.
• Conduct in-service education related to MDS, RAPS, Triggers, PDPM, and Medicare requirements.
• Maintain strict adherence to MDS and care-planning schedules, including non-Medicare residents.
• Ensure compliance with state and federal regulations.

Salary: $46 - $51 (based on experience)

Position Overview

Palm Beach Nursing Center is seeking an experienced and detail-oriented MDS Director(RN/LPN) to oversee the MDS process and ensure regulatory compliance while promoting exceptional resident-centered care. This leadership role is critical in supporting clinical excellence, care planning accuracy, and interdisciplinary collaboration.
