Job summary

Palm Beach Nursing Center in Lake Worth Beach, FL, is seeking an experienced MDS Director (RN/LPN) for a full-time role. You will oversee MDS processes, ensure regulatory compliance, and promote resident-centered care through strong leadership and collaboration with interdisciplinary teams.

The position requires Florida RN or LPN licensure and at least 3 years as an MDS Coordinator in a skilled nursing setting, with expertise in MDS, PDPM, and Medicare guidelines.

Qualifications

• Active Florida RN or LPN license in good standing.
• Minimum 3 years’ MDS Coordinator experience in SNF.
• Strong knowledge of MDS, PDPM, Medicare guidelines and care planning.

Responsibilities

• Oversee MDS process and ensure regulatory compliance.
• Lead interdisciplinary team meetings and care planning.
• Monitor accuracy and timeliness of MDS assessments and documentation.

Now Hiring: MDS Director (RN/LPN)
Facility: Palm Beach Nursing Center
Location: Lake Worth Beach, Florida
Position Type: Full-Time
Salary: $46 - $51 (based on experience)

Position Overview

Palm Beach Nursing Center is seeking an experienced and detail-oriented MDS Director(RN/LPN) to oversee the MDS process and ensure regulatory compliance while promoting exceptional resident-centered care. This leadership role is critical in supporting clinical excellence, care planning accuracy, and interdisciplinary collaboration.

Key Responsibilities

• Ensure the delivery of high-quality, compliant care for long-term care residents
• Monitor accuracy and timeliness of MDS assessments and documentation
• Conduct comprehensive initial assessments and resident interviews
• Develop and oversee individualized care plans with both short-term and long-term goals
• Lead and participate in interdisciplinary team meetings
• Collaborate with caregivers, families, and clinical specialists to optimize resident outcomes
• Provide leadership and guidance to nursing staff
• Conduct in-service education related to MDS, RAPS, Triggers, PDPM, and Medicare requirements
• Maintain strict adherence to MDS and care-planning schedules, including non-Medicare residents
• Ensure compliance with state and federal regulations

Qualifications

• Licensure: Active Florida RN or LPN license in good standing
• Experience: Minimum of 3 years’ experience as an MDS Coordinator in a Skilled Nursing Facility
• Knowledge Base: Strong working knowledge of MDS, PDPM, Medicare guidelines, and care-planning processes
• Skills: Excellent organizational, leadership, and communication skills
