Job summary

Rialtohealth in California is seeking a Director of MDS to lead the development, completion and transmission of resident assessments in alignment with state regulations. You will ensure care plans include measurable objectives, supervise MDS staff, coordinate interviews, and maintain timely assessments.

CA RN licensure, CPR certification, and 2–4 years of direct care in SNF with MDS experience are required.

Qualifications

• CA RN license required with CPR certification.
• 2–4 years' direct care experience preferred in SNF.
• Previous MDS experience and supervisory background desired.

Responsibilities

• Lead development, completion and transmission of resident MDS.
• Ensure care plans have measurable objectives and timetables.
• Coordinate interviews and update assessments quarterly/annually.
• Supervise MDS staff and ensure regulatory compliance.
