Premier Rehab & Healthcare at Burlington is seeking an experienced MDS Coordinator to join our team. The MDS Coordinator plays an essential role in ensuring accurate resident assessments, comprehensive care planning, regulatory compliance, and high-quality, resident-centered care.

Qualifications

• Must hold Vermont RN or LPN license.
• MDS and PPS experience is required.
• Care planning experience is preferred.
• Strong knowledge of MDS, PPS and regulatory requirements.
• Excellent assessment, documentation, organizational and communication skills.
• Strong attention to detail and ability to meet deadlines.
• Ability to work effectively as part of an interdisciplinary team.
• Commitment to providing high-quality, resident-centered care.

Responsibilities

• Coordinate development and completion of comprehensive resident assessments.
• Complete MDS assessments in accordance with state and federal regulations.
• Conduct and coordinate Care Area Assessments (CAAs).
• Coordinate and support individualized resident care plans.
• Collaborate with nursing, therapy, social services, dietary, physicians and other team members.
• Ensure MDS documentation is accurate, complete, and submitted within timelines.
• Monitor MDS schedules and regulatory deadlines.
• Participate in facility survey preparation and survey process.
• Review resident documentation for accuracy and completeness.
• Communicate effectively with residents, families and interdisciplinary team members.
• Support quality improvement and regulatory compliance initiatives.
