Job summary

MB Healthcare is seeking an RN with MDS Coordinator certification to join our Nursing department at Clover Meadows Healthcare and Rehabilitation Center. The role focuses on resident assessments, care planning, and regulatory compliance in a Skilled Nursing Facility.

The ideal candidate will have an active RN license, MDS certification, and strong clinical assessment skills, with knowledge of Medicare/Medicaid guidelines. Full-time on-site position with competitive pay and benefits.

Qualifications

• RN license required.

• Clinical experience in a nursing facility.
• Demonstrated clinical assessment skills.
• Knowledge of Medicare and Medicaid regulations for skilled nursing.

Responsibilities

• Ensure development and completion of resident assessments per state requirements and facility policies.
• Promote resident well-being; review care plans and analyze financial coverage to ensure needs are met.
• Utilize Resident Assessment Instrument (RAI) for data collection from interviews.
• Cover areas such as mood, behavior, cognition, and nutrition in assessments.
• Coordinate with social services, dietitians, rehab, and medical staff to implement care plans.
• Implement and monitor care plans for effectiveness; ensure compliance with Medicare requirements.
