Job summary

Clover Meadows Healthcare and Rehabilitation Center is seeking an experienced RNAC MDS Coordinator to join our Nursing team in a full-time role. The ideal candidate will be an RN with MDS coordinator certification, strong analytical and clinical assessment skills, and working knowledge of Medicare and Medicaid guidelines as they apply to skilled nursing facilities.

Qualifications

• Registered Nurse with current license and active credentials.
• MDS Coordinator certification required.
• Experience in a nursing facility.
• Knowledge of Medicare and Medicaid regulations.

Responsibilities

• Develop and complete resident assessments per state requirements.
• Promote resident well-being and create/review care plans.
• Gather information via interviews using the RAI.
• Assess mood, behavior, cognition, and nutrition.
• Coordinate with social services, dietitians, rehab, and medical staff.
• Implement and monitor care plans; ensure compliance with Medicare standards.
