Qualifications

• Active, unencumbered RN license in Illinois.
• Minimum 2+ years as an MDS Coordinator in SNF or long-term care.
• RAC-CT certification preferred.
• Deep understanding of CMS RAI guidelines, Medicare/Medicaid, PDPM, and state regulations.
• Experience with PCC or similar EHR systems.

Responsibilities

• Coordinate, schedule, and complete all required MDS assessments (OBRA and PPS) accurately and in strict compliance with federal and state timelines.
• Facilitate interdisciplinary care plan meetings, working with nursing, therapy, social services, and dining teams to establish individualized care plans.
• Monitor and optimize PDPM coding and reimbursement to reflect care delivered.
• Participate in weekly Triple Check meetings prior to billing submission; perform routine audits of clinical documentation.
• Educate floor nurses and interdisciplinary team members on documentation standards and MDS coding requirements.
