Qualifications

MDS Coordinator (RN or LPN) - PT - Part-Time 1st Shift

Position Summary

Develops, plans, coordinates, organizes, directs, and evaluates the MDS and Care Planning activities/systems for the campus in accordance with scope of practice, doctors' orders, established company policies and procedures, standards, and guidelines; current federal, state, and local laws and regulations governing the nursing department and campus; and as directed by the supervisor by performing the following duties personally or through subordinate team members.

In this role, you will:

  1. Develop, supervise, plan, coordinate, organize, direct, evaluate, and monitor the MDS, CAAs, and care planning process in the campus.
  2. Assist with the resident admission process as designated by the Director of Nursing.
  3. Complete administrative duties such as forms, inspections, reports, OSCAR reports, assessments, MDS', CAAs, care plans, progress notes, and similar items as assigned.
Qualifications
  • Graduate from an accredited school of Nursing approved by the State of Nebraska.
  • Possess a valid RN or LPN license from the appropriate state, including maintaining required CEUs.
  • RN license preferred, but LPN candidates will also be considered.
  • Experience in Long Term or Acute Care and MDS/RAPS/Care Planning.
  • Willing and able to become CPR and first aid certified.
