Job summary

Resolve Harmony Center in Arizona is seeking an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as an MDS Coordinator. The role focuses on accurate assessments, regulatory compliance, and appropriate reimbursement within our skilled nursing facility.

You will complete and coordinate MDS assessments, ensure timely submissions, support the care planning process, and monitor quality measures.

Qualifications

• Current RN or LPN license in good standing.
• Must have AZ Fingerprint Clearance Card.
• Experience as MDS Coordinator in a skilled nursing facility preferred.
• Strong understanding of Medicare, Medicaid, PDPM, and MDS regulations.

Responsibilities

• Complete and coordinate MDS assessments in accordance with state and federal regulations.
• Ensure accuracy and timeliness of MDS submissions and supporting documentation.
• Coordinate the interdisciplinary care planning process.
• Review clinical documentation to ensure appropriate reimbursement and regulatory compliance.
• Monitor quality measures and identify opportunities for improvement.
• Collaborate with nursing leadership, therapy, social services, and other departments.
• Assist with Medicare and Medicaid case management related to resident assessments.
• Maintain knowledge of current regulations and reimbursement guidelines.
• Support survey readiness and compliance initiatives.
