Job summary

iCare Health Network is seeking an RN MDS Coordinator to oversee Medicare/Medicaid case-mix documentation, MDS completion, and coordination across Nursing, Dietary, Social Services, and Rehabilitation for proper reimbursement.

The role requires an active CT RN license and knowledge of PPS/MDS processes, with preferred supervisory experience in long-term care and at least 3 months of MDS-related training or experience.

Qualifications

• Active RN license in the State of Connecticut is required.
• Knowledge of Quality Assurance/Improvement processes is required.
• Bachelor’s or Associate degree in a health care related field is preferred.
• Experience in a supervisory position in long-term care is preferred.
• Requires at least 3 months experience completing MDS and completion of MDS training or equivalent knowledge of PPS.

Responsibilities

• Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinates and facilitates care conferences with the interdisciplinary team.
• Coordinates, completes and reviews Minimum Data Sets (MDS).
• Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS process.
• Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced.
• Cross trains between PPS and OBRA.
• Conducts regular resident interviews as required by the MDS.
• Responds to clinical emergencies, regulatory visits, surveys and issues outside of normal work hours as needed.

RN MDS Coordinator

The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement.

Responsibilities:

• Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process.
• Coordinates and facilitates care conferences with the interdisciplinary team.
• Coordinates, completes and reviews Minimum Data Sets (MDS)
