Oak River Rehab is seeking an experienced MDS Coordinator for our Anderson, California location. The role requires a current LVN or RN license in good standing, with knowledge of MDS assessments, care planning, and reimbursement regulations.

Pay starts at $45.00 per hour, depending on experience. The MDS Coordinator will coordinate and complete MDS assessments, develop and update resident care plans, and collaborate with nursing, therapy, dietary, and social services to ensure regulatory

Qualifications

• Current LVN or RN license in good standing.
• Experience with MDS assessments and care planning preferred.
• Knowledge of Medicare, Medicaid, and reimbursement regulations.
• Strong organizational and communication skills.

Responsibilities
