Woburn Rehabilitation and Nursing Center is seeking an MDS Coordinator to join our care team in Woburn, MA. The role focuses on coordinating resident assessments, developing care plans, and ensuring high-quality, compliant care with an emphasis on regulatory requirements.

Ideal candidates are experienced RNs or LPNs with MDS and PPS experience, strong documentation skills, and the ability to work effectively with an interdisciplinary team in a resident-centered setting.

Qualifications

• Massachusetts RN or LPN license required
• MDS and PPS experience (2+ years)
• Care planning experience required
• Strong assessment and documentation skills
• Excellent communication and interpersonal skills
• Knowledge of MDS/PPS regulations
• Ability to work with interdisciplinary team

Responsibilities

• Coordinate development of comprehensive resident assessments
• Perform assessments per state and federal rules
• Implement Care Area Assessments (CAAs)
• Support development of individualized resident care plans
• Collaborate with interdisciplinary care team
• Assist with survey readiness and process
• Ensure MDS documentation is accurate and timely
• Communicate with nursing staff, physicians, therapists and social workers
• Ensure regulatory compliance
