Diversicare Foley is seeking an MDS Coordinator (RNAC or LNAC) to join our dedicated care team in Foley, AL. You will oversee the RAI process, ensuring accuracy and compliance with state and federal regulations, and coordinate care plans with the interdisciplinary team.

Qualifications

• Two years of MDS experience preferred, but not required.
• Current LPN or RN license in the state of employment.
• Working knowledge of the MDS 3.0.

Responsibilities

• Coordinate the RAI Process with accuracy and regulatory compliance.
• Collaborate with the interdisciplinary team to assess needs and care plans.
• Conduct Care Plan conferences with patients, residents, and families.
• Educate on the RAI Process and ensure accurate MDS coding.
• Monitor Quality Measures and reflect quality standards in MDS.
• Maintain documentation and timely submission to state databases and entities.
• Ensure compliance with Medicare and Medicaid guidelines.
