Diversicare Foley is seeking an MDS Coordinator to join our dedicated team at Foley, AL. The role focuses on ensuring accurate and compliant MDS assessments, coordinating the RAI process, and supporting care plans for residents.

RN or LPN licensure in the state is required, with MDS 3.0 knowledge. The coordinator will lead care plan conferences, uphold regulatory standards, and collaborate with the interdisciplinary team to improve resident outcomes.

Qualificações

• Two years of MDS experience preferred but not required.
• Current LPN or RN license in the state of employment.
• Working knowledge of the MDS 3.0.

Responsabilidades

• Coordinate the RAI Process with accuracy and regulatory compliance.
• Collaborate with the inter-disciplinary team to assess needs and plan care.
• Conduct Care Plan conferences with patients, residents and families.
• Educate on the RAI Process and ensure proper MDS coding.
• Monitor Quality Measures and maintain MDS accuracy for quality standards.
• Maintain documentation and timely submission to state databases and entities.
• Ensure compliance with Medicare and Medicaid guidelines.
