• Coordinate the RAI Process to ensure accuracy and compliance.
• Collaborate with interdisciplinary team to assess needs and coordinate care plans.
• Conduct Care Plan conferences with patients, residents, and families.
• Provide education related to the MDS process and accurate coding of assessments.
• Monitor quality measures and ensure MDS accuracy reflects quality standards.
• Maintain documentation and timely submission to state databases and other entities.
• Ensure compliance with Medicare and Medicaid regulatory guidelines.
