Alpine Care of St. Charles in St. Charles, IL is seeking a dedicated MDS Coordinator RN/LPN to join our care team. You will coordinate and implement the MDS assessment process, ensuring accurate data and regulatory compliance.

The ideal candidate has an IL license (RN or LPN), at least 1 year of healthcare experience, and strong communication and organizational skills. You will educate staff, collaborate with disciplines, and help improve care quality.

Qualifications

• Current IL RN or LPN license is required.
• Minimum 1 year healthcare experience, long-term care or rehabilitation preferred.
• Strong knowledge of MDS processes and regulations.
• Excellent communication and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Strong attention to detail and organizational skills.

Responsibilities

• Conduct MDS assessments and reviews to ensure accurate data collection and reporting.
• Coordinate with interdisciplinary team members to gather information and complete MDS assessments.
• Ensure MDS assessments are completed timely and in compliance with regulatory requirements.
• Develop and implement strategies to improve MDS accuracy and completeness.
• Provide education and training to staff on MDS processes and procedures.
• Maintain accurate and up-to-date records and reports.
• Collaborate with other departments to ensure seamless communication and coordination.
