Job summary

Aviata Health Group is seeking a qualified RN or LPN MDS Coordinator to join our family in Tampa, FL. You will oversee the coordination and completion of the MDS, CAA’s, and Care Plan in line with Federal and State regulations, ensuring high-quality resident care.

Responsibilities include coordinating MDS-related processes, monitoring Medicare/Managed Care residents, and driving improvements in care delivery to achieve superior results.

Qualifications

• Active state RN or LPN license required.
• 6 months of MDS Coordinator experience.
• Certification RNAC or RAC-CT preferred.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.

Responsibilities

• Oversee coordination and completion of the MDS, CAA’s and Care Plan per Federal and State regulations.
• Monitor and document Medicare/Managed Care resident care in collaboration with facility team.
• Develop and implement system improvements to enhance care processes and outcomes.
