Adept Nursing & Rehab of Grand Island is hiring a full-time MDS Coordinator. The role requires an RN or LPN license and involves managing MDS processes, coding, and regulatory compliance for residents.

You will collaborate with nursing and admissions teams to ensure accurate assessments and appropriate placements. Ideal candidates will have 2+ years in MDS coordination or related roles, with experience in Medicare/Medicaid guidelines.

Qualifications

• Valid RN or LPN license.
• 2+ years of experience in MDS coordinator role or related field preferred.
• MDS, Medicare, and Medicaid experience preferred.

Responsabilités

• Gather information on current and future residents for MDS assessment.
• Participate in admission process and assess nursing needs for placement.
• Determine RUGs and potential admission expenses.
• Review applications for admission and communicate assessment results.
• Complete and assure accuracy of the MDS process for residents.
• Maintain knowledge of Medicare criteria and inform nursing staff of changes.
• Monitor Case Mix Index (CMI) for Medicaid reimbursement risks.
• Facilitate problem-solving for complex admissions.
• Monitor Medicare assessment schedules and documentation for accuracy.
