Job summary

Liberty Health's Liberty Commons of Alamance County in Burlington, NC is seeking an experienced MDS Coordinator (RN). You will manage MDS timelines, coordinate with disciplines, ensure accuracy, complete MDS and inputs, and secure timely state approvals.

The role requires an active NC RN license, expertise in MDS-RAP and Care Planning, and familiarity with RUG-IV and MDS 3.0. Join a team focused on compassionate care and compliance.

Qualifications

• Registered Nurse with a current NC RN license.
• Experience with MDS-RAP and Care Planning.
• Preference for RUG-IV and MDS 3.0 familiarity.

Responsibilities

• Maintains and follows a schedule of due dates for all MDS.
• Coordinates the completion of the MDS by all disciplines.
• Reviews the MDS data for accuracy with staff as needed.
• Completes the MDS and inputs into the computer.
• Verifies timely state transmission and approval.
• Corrects rejected records and resubmits as needed.
• Completes CAAs per state guidelines.
• Formulates Resident Care Plan with interdisciplinary team.
• Schedules and conducts RCP meetings regularly.
• Communicates RCP to staff and reports noncompliance.
• Works with departments to ensure RCP compliance.
• Performs other duties as directed by DON/Administrator.

We are currently seeking an experienced:

MDS COORDINATOR (RN LICENSE REQUIRED)

Job Description:

• Maintains and follows a schedule of due dates for all MDS.
• Coordinates the completion of the MDS by all disciplines.
• Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
• Completes the MDS and inputs into the computer.
• Verifies that assessments have been transmitted and approved by the State in a timely manner.
• Corrects any rejected records and prepares them for re-submission.
• Completes CAAs according to state guidelines.
• Formulates Resident Care Plan (RCP) along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame.
• Schedules and conducts RCP meetings on a regular and timely basis.
• Communicates the RCP to appropriate staff members and notifies supervisor if noncompliance by staff is noted.
• Works with all in house and ancillary departments to assure understanding and compliance with the RCP.
• Performs other related duties as directed by the DON and -or Administrator.

Job Requirements:

• Registered Nurse, graduated from an accredited School of Nursing and have a current, valid RN license from the North Carolina Board of Nursing and submit proof of license renewal every other year.
• Ability to read, know, and follow personnel, department and facility policies and procedures and adhere to local state and federal requirements.
• Experience with MDS-RAP and Care Planning functions.
• Prefer experience with RUG-IV.
• Prefer experience with MDS 3.0.
