Liberty Health in Winston-Salem, NC seeks an experienced MDS Coordinator (RN license required) to lead MDS administration across disciplines and ensure timely, accurate documentation. The role involves coordinating MDS completion, validating data, and ensuring state compliance, with strong collaboration with care teams.

The position requires NC RN licensure, knowledge of MDS-RAP, Care Planning, and preferably RUG-IV and MDS 3.0 experience.

• Preference for experience with RUG-IV and MDS 3.0.

Responsabilités

• Maintain and follow a schedule of due dates for all MDS.
• Coordinate completion of MDS by all disciplines.
• Review MDS data for accuracy and liaise with staff to ensure precision.
• Complete MDS and input into the computer system.
• Verify timely transmission and State approval of assessments.
• Correct rejected records and re-submit as needed.
• Complete CAAs per state guidelines.
• Formulate Resident Care Plan (RCP) with interdisciplinary team.
• Schedule and conduct regular RCP meetings.
• Communicate RCP to staff and report noncompliance to supervisor.
• Collaborate with in-house and ancillary departments on RCP understanding and compliance.
• Perform other related duties as directed by DON/Administrator.

We are currently seeking an experienced:

MDS COORDINATOR (RN LICENSE REQUIRED)

Job Description:

• Maintains and follows a schedule of due dates for all MDS.
• Coordinates the completion of the MDS by all disciplines.
• Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
• Completes the MDS and inputs into the computer.
• Verifies that assessments have been transmitted and approved by the State in a timely manner.
• Corrects any rejected records and prepares them for re-submission.
• Completes CAAs according to state guidelines.
• Formulates Resident Care Plan (RCP) along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame.
• Schedules and conducts RCP meetings on a regular and timely basis.
• Communicates the RCP to appropriate staff members and notifies supervisor if noncompliance by staff is noted.
• Works with all in house and ancillary departments to assure understanding and compliance with the RCP.
• Performs other related duties as directed by the DON and -or Administrator.

Job Requirements:

• Registered Nurse, graduated from an accredited School of Nursing and have a current, valid RN license from the North Carolina Board of Nursing and submit proof of license renewal every other year.
• Ability to read, know, and follow personnel, department and facility policies and procedures and adhere to local state and federal requirements.
• Experience with MDS-RAP and Care Planning functions.
• Prefer experience with RUG-IV.
• Prefer experience with MDS 3.0.
