Yardley Rehabilitation and Healthcare Center in Yardley, PA, is seeking an MDS Coordinator to ensure timely, accurate, and compliant completion of all MDS assessments. Full-time and part-time opportunities available.

The role emphasizes regulatory compliance, reimbursement support, data validation, audits, and collaboration with the DON and care teams to accurately represent resident needs. Qualifications include an accredited nursing degree (RN/BSN), active RN license, and at least three years

Competenze

Three years of clinical experience in long-term care.
Prior MDS/RAI experience required.
Strong clinical assessment skills with attention to detail.

Mansioni

Ensure timely and accurate completion of all MDS assessments per regulations and deadlines.
Verify compliance with federal, state, and facility regulations related to MDS documentation and submission.
Supervise MDS data entry, validation, and transmission processes for accuracy and timeliness.
Identify and resolve data discrepancies and validation issues.
Prepare and present MDS-related reports to the DON and leadership team.
Support and participate in audits, surveys, and regulatory reviews.
