Southern Hills Specialty Care in Osceola, IA is hiring a MDS Coordinator (RN) to lead comprehensive resident assessments and care planning. The role focuses on timely MDS processes, regulatory compliance, and collaboration with an interdisciplinary team to improve care quality.

The RN will ensure accurate documentation, support CMS compliance, and participate in on-call rotations as part of leadership duties in this 90-bed skilled nursing community.

Qualifications

• Active RN license in good standing (Iowa).
• Current CPR certification.
• Knowledge of CMS/state/federal long-term care regulations.
• Ability to collaborate with interdisciplinary teams.

Responsibilities

• Complete and coordinate MDS assessments to evaluate resident needs.
• Develop and update individualized care plans with the team.
• Monitor documentation for accuracy and compliance with regulations.
• Participate in care conferences and family communications.
• Support audit readiness and maintain survey-ready documentation.
