Job Description

As an RN MDS Coordinator, you will oversee Medicare requirements and MDS coordination, ensuring timely and accurate submissions. You will collaborate with clinical staff, coordinate care plan schedules, and serve as a resource for MDS and PPS processes. Your responsibilities include documentation, data monitoring, resident care planning, and electronic submissions to the state of Michigan.

Minimum Requirements

• RN license
• Associate’s degree in nursing
• Current CPR and BLS certification or obtain via onsite training
• MDS Certification or plan to achieve it with site support
• 2+ years of RN experience in a skilled nursing facility or 5+ years in a direct care role (LPN, CNA)
• 3+ years of experience as an MDS coordinator
