Job summary
Kenneth Moy DuPage Care Center in DuPage County is seeking a committed MDS Coordinator to oversee federally mandated assessments, collaborate with the interdisciplinary team, and ensure accurate chart review and care planning. This role requires an Illinois RN license and MDS experience, with RAC-CT certification pursued within six months.

Qualifications
- Diploma in Nursing from an accredited college or university.
- One year experience working with MDS.
- Current IL RN license in good standing.
- AANAC RAC-CT certification required within six months (reimbursed).
Responsibilities
- Coordinate data collection and input for federally mandated MDS assessments.
- Monitor input for accuracy, timeliness, and reimbursement standards.
- Review clinical charts and condition of residents.
- Plan and coordinate care with the interdisciplinary team.
- Discuss resident condition with physicians and family; lead care-plan meetings.
- Complete the MDS computer database; ensure Medicare/IDPH compliance.
- Stay current with regulations and professional developments.
- Perform RN duties as assigned.
