Water’s Edge at Port Jefferson for Rehabilitation and Nursing is seeking an MDS Coordinator RN to lead the resident assessment process, ensuring compliance with federal and state regulations and company procedures. The role acts as in-house case manager, coordinating care with physicians, families, and third-party payers, and tracking ARD, RUG scores, and care plans to optimize outcomes.

Applicants must hold an active RN license and have LTC experience; previous MDS coordination is preferred.

Qualifications

• RN license in state of practice.
• Minimum 2 years of clinical experience in a healthcare setting.
• Minimum 1 year of experience in a long-term care setting.
• Prior experience as an MDS coordinator is preferred.
• Training program available for RN candidates with demonstrated assessment skills.

Responsibilities

• Oversees accurate and thorough completion of the MDS, CAAs and Care Plans per regulatory guidelines.
• Acts as in-house case manager coordinating care with physicians, families, and payers.
• Identifies regulatory risks and coordinates resource use and clinical trends with leadership.
• Calculates ARD and RUG scores to reflect provided care and services.
• Ensures timely electronic submission of MDS to the state database and follows up on validations.
• Participates in committees and educates IDT members about RAI processes.
