Job summary

Andover Forest Post Acute Care Center is seeking a full-time MDS Coordinator RN to oversee the MDS process, coordinate comprehensive assessments, and ensure accurate clinical documentation supporting reimbursement. You'll collaborate with nursing, therapy, social services, and the interdisciplinary team to ensure residents' needs are accurately assessed and documented.

Active MA RN license and MDS experience required; direct hire, not agency.

Qualifications

• Active Massachusetts RN license required.
• MDS experience in skilled nursing or long-term care setting.
• Strong knowledge of MDS assessments, care planning, and reimbursement.
• Excellent organizational and communication skills.
