Job summary

St. Patrick's Home Rehabilitation and Health Care is actively seeking an MDS Coordinator for our Skilled Nursing Facility in the Bronx, NY. The role focuses on accurate MDS assessments, care plans, and ongoing plan management within a collaborative team.

The ideal candidate holds a NYS RN license and has prior MDS 3.0 experience, with strong organizational and communication skills, and a commitment to resident confidentiality and high-quality documentation.

Qualifications

• Must have prior experience with MDS 3.0.
• Valid NYS Registered Nurse (RN) License.
• Long Term Care experience required.
• Must be highly organized, professional and motivated.
• Should have solid computer skills.
• Excellent communication skills.
• Should be friendly and a team worker.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining and updating all care plans and assessments as required.
• Monitoring and auditing clinical records, ensuring accuracy and timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of resident and facility information at all times.
