Job summary

St. Patrick's Home Rehabilitation and Health Care in the Bronx, NY is seeking a dedicated MDS Coordinator to manage MDS 3.0 processes, care plans, and clinical documentation for residents in our Skilled Nursing Facility.

Responsibilities include completing assessments, coordinating monthly calendars, maintaining plans, and auditing records to ensure accuracy while protecting resident confidentiality. Collaboration with the DON and care teams is essential to ensure quality resident care.

Qualifications

• Must have prior experience with MDS 3.0.
• Valid NYS RN License.
• Long Term Care experience required.
• Highly organized, professional & motivated.
• Solid computer skills.
