Timbercreek Rehab and Healthcare Center is seeking an experienced MDS Coordinator (RN) to ensure timely and accurate completion of the RAI process and care management from admission through discharge. Coordinate with the Director of Nursing and VP of Clinical Reimbursement to align with federal, state, and certification guidelines.

The role requires active RN licensure and prior MDS experience, with preference for LTC/SNF/AL/MC settings.

Qualifications

• Active RN license in the practicing state.
• Prior MDS experience is highly preferred.
• Experience in LTC/SNF/AL/MC settings is highly preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and the Minimum Data Set.

Responsibilities

• Ensure timely and accurate completion of the RAI process from admission to discharge.
• Manage care planning and care management processes per policy and guidelines.
• Coordinate information systems operations and education for the clinical department under supervision.

Do You Have What It Takes?

• A fire and passion for working with seniors
• A flexible, fun, and energetic personality
• Currently licensed as a Registered Nurse (RN) in the state in which you are practicing
• Prior MDS experience highly preferred
• Minimum of one year of experience in a LTC/SNF/AL/MC setting highly preferred
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement highly preferred
• Must possess a thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set
• Knowledge of the care planning process required
• Professional image in both appearance and behavior
• Excellent written and oral communication skills
