Job summary

Timbercreek Rehab and Healthcare Center is seeking an RN MDS Coordinator to manage RAI and care coordination from admission through discharge. You will align with policy, federal/state guidelines, and reimbursement processes to ensure accurate data tracking.

The ideal candidate is an RN with MDS experience and prior LTC/SNF/AL/MC exposure, plus strong communication and professional presentation.

Qualifications

• Must be a Registered Nurse licensed in the state of practice.
• MDS experience strongly preferred.
• Minimum of one year in LTC/SNF/AL/MC setting preferred.
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement preferred.
• Understanding OBRA, Quality Indicator, and Minimum Data Set.

Responsibilities

• Complete RAI and care management processes from admission to discharge.
• Coordinate information systems operations and education for the clinical department.
• Ensure compliance with company policy and Federal/State guidelines.

What Will You Do in This Role?

As a MDS Coordinator | RN, you will be responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge, and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and education for the clinical department.

Do You Have What It Takes?

• A fire and passion for working with seniors
• A flexible, fun, and energetic personality
• Currently licensed as a Registered Nurse (RN) in the state in which you are practicing
• Prior MDS experience highly preferred
• Minimum of one year of experience in a LTC/SNF/AL/MC setting highly preferred
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement highly preferred
• Must possess a thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set
• Knowledge of the care planning process required
• Professional image in both appearance and behavior
• Excellent written and oral communication skills
