Qualifications

• Experience as an MDS Nurse is preferred.

Responsibilities

• Complete MDS, CAA’s, and care plans within regulated timeframes
• Coordinate scheduling of the RAI process with the interdisciplinary team
• Assess residents through physical exams, interviews, and chart reviews
• Discuss care needs with caregivers, including physicians and staff
• Review information from hospitals and outside agencies for assessments and care planning
• Identify and initiate significant changes in MDS assessments
• Conduct PPS meetings, maintain assessments per Medicare schedule, and monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Stay current with AANAC requirements

Qualifications

• RN or LPN licensure
• Knowledge of the RAI process and PPS principles (strongly preferred)
• Experience as an MDS Nurse
