Job summary

Batavia Rehab and Healthcare Center is seeking an experienced MDS Coordinator (RN) to manage the RAI process from admission to discharge, ensuring accuracy and compliance with federal, state, and certification guidelines.

The ideal candidate holds an RN license, has MDS experience, and is passionate about senior care. This role requires teamwork with the Director of Nursing and provides competitive pay and benefits.

Qualifications

• Currently licensed as an RN in the practicing state.
• Prior MDS experience preferred.
• Experience in LTC/SNF/AL/MC settings preferred.
• Knowledge of Case-Mix and Medicare PPS processes.
• Understanding OBRA and MDS requirements.
• Familiar with care planning processes.
• Strong written and verbal communication.

Responsibilities

• Ensure timely, accurate completion of the RAI and care management processes from admission to discharge.
• Coordinate information systems operations and education for the clinical department as needed.

What Will You Do in This Role?

As a MDS Coordinator | RN, you will be responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge, and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and education for the clinical department.

Do You Have What It Takes?

• A fire and passion for working with seniors
• A flexible, fun, and energetic personality
• Currently licensed as a Registered Nurse (RN) in the state in which you are practicing
• Prior MDS experience highly preferred
• Minimum of one year of experience in a LTC/SNF/AL/MC setting highly preferred
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement highly preferred
• Must possess a thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set
• Knowledge of the care planning process required
• Professional image in both appearance and behavior
• Excellent written and oral communication skills
