Job summary

Signature HealthCARE North Hardin Rehab & Wellness Center is seeking a detail-oriented MDS candidate to conduct resident assessments, coordinate with the interdisciplinary team, and ensure compliant PPS processes.

The ideal candidate has RN or LPN licensure with required state credentials, at least 1 year of MDS experience, and a commitment to high-quality resident care in a long-term care setting.

Qualifications

• The ideal candidate is very detailed oriented, organized and self-motivated, and remains bedside for resident assessment and family meetings.
• Knowledge of nursing and medical practices, regulations and guidelines for long-term care is required.

Responsibilities

• Collect information to complete the MDS using medical records, bedside assessment, and staff/family interviews.
• Attend Daily PPS meetings, weekly Medicare meetings, and month-end meetings to ensure federal billing requirements are met.
• Educate the interdisciplinary team on the RAI process and changes as appropriate.
