We are now accepting applications for the MDS Coordinator - RN position.

Position Summary:

The Care Manager is responsible for developing, maintaining, and implementing nursing practice standards and objectives related to the Minimum Data Set (MDS) requirements, in accordance with policies and procedures of the governing body, and in compliance with federal, state, and local regulations. The role includes ensuring optimal reimbursement through proper documentation and diagnoses to justify skilled services and reimbursement requests. Skilled nursing experience is desired.
