Pride Health is hiring a Clinical Care Coordinator RN to join our integrated care team. This remote field-based role supports members across Worcester, MA, and surrounding areas. You’ll spend approximately 50% of your time in the field, visiting members in their homes or long-term care facilities to provide high-quality, person-centered care, and the other 50% working remotely.

Shift: M-F

Pay: $40-45 Hourly

Contract: 6 months, 6/30-12/29

Key Responsibilities

  • Conduct comprehensive needs assessments (in-person and/or telephonically) to evaluate members’ medical, behavioral, functional, cultural, and socioeconomic needs.
  • Develop and implement individualized care strategies that prioritize appropriate and least restrictive levels of care.
  • Initiate and coordinate referrals to social service programs, including financial, psychosocial, and community-based services.
  • Serve as the primary point of contact to manage the care plan across the continuum, collaborating with internal care teams, providers, and community partners.
  • Ensure timely and accurate communication of health-related information to support quality and coordinated service delivery.
  • Maintain accurate and up-to-date documentation of all care activities and interactions.

Required Qualifications

  • Current, unrestricted RN license in the state of Massachusetts
  • 2+ years of clinical experience
  • Proficiency in Microsoft Office (Word, Excel, Outlook)
  • Access to reliable transportation and willingness to travel regularly throughout the assigned region
  • Valid U.S. driver's license
  • Bachelor’s or Master’s degree in Nursing preferred
  • 1+ years of community case management experience with individuals who have complex health needs
