Are you an experienced MDS Registered Nurse interested in the next step? The MDS Coordinator provides oversight of the RAI process, conducts assessments, and coordinates care plans for guests. The MDS Coordinator supervises the Care Management Nurse and MDS Nurse.

Responsibilities

• Complete the MDS, CAA’s, and care plans within regulated timeframes.
• Coordinate scheduling of the RAI process with the interdisciplinary team.
• Assess residents through physical assessments, interviews, and chart reviews.
• Discuss resident care needs with caregivers, including physicians, nursing, social services, therapy, dietary, and activity staff.
• Review information from hospitals, consultants, and outside agencies for assessments and care planning.
• Coordinate, identify, and initiate significant changes in MDSs.
• Conduct PPS meetings, maintain MDS assessments per Medicare schedule, and monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
• Stay current with American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications

• Registered Nurse (RN)
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment System (PPS) — strongly preferred.
• Experience as an MDS Nurse
