Briarwood Rehabilitation and Healthcare Center is seeking a dedicated MDS Coordinator to oversee accurate MDS/RAI assessments in a 120-bed long-term care setting. The role ensures regulatory compliance, timely data entry, and collaboration with the care team.

The ideal candidate holds an active RN/BSN/LPN license with 3+ years in long-term care and strong knowledge of Medicare/Medicaid regulations. This full-time on-site position offers growth and a supportive work culture.

Qualifications

• RN/BSN/LPN required with active license.
• Minimum 3 years of long-term care experience.
• MDS/RAI experience required and knowledge of Medicare/Medicaid.

Responsibilities

• Ensure timely, accurate MDS assessments per regulations.
• Supervise MDS data entry, validation, and transmission.
• Identify and resolve data discrepancies and validation issues.
• Prepare MDS reports for DON and leadership.
• Support audits and regulatory reviews and respond to inquiries.
