Avante at Orlando Skilled Nursing and Rehabilitation Center is seeking an experienced MDS Coordinator to oversee resident assessments and ensure compliance with federal, state, and local regulations. You will work with the Interdisciplinary Care Team to maintain high-quality care and Five-Star performance.

The role requires an active LPN/RN license in Florida, a nursing degree, and 2+ years in healthcare settings. Join a compassionate team dedicated to excellence in patient care and education.

Qualifications

• Active, unencumbered LPN or RN license in Florida.
• Nursing degree/diploma from an accredited program.
• 2+ years of experience in a hospital, skilled nursing, or healthcare setting preferred.

Responsibilities

• Coordinate and conduct MDS assessments and care planning in compliance with regulations.
• Submit MDS assessments timely to State repositories per RAi guidelines.
• Collaborate with Interdisciplinary Care Team to set review dates.
• Update resident care plans to reflect health changes.
• Monitor Quality Measures Reports to maintain high Five-Star ratings.
• Educate nursing staff, residents, and families to develop personalized care plans.
• Participate in surveys and inspections by regulatory agencies.
• Maintain confidentiality and ensure patient privacy.
