Trinity Regional Rehab Center is seeking an experienced MDS Coordinator RN to oversee the Resident Assessment Instrument (RAI) process, ensuring accurate MDS submissions and supporting care planning for residents.

The role requires strong knowledge of OBRA, CMS guidelines, and collaboration with the interdisciplinary team to optimize patient outcomes and ensure timely assessments.

Qualifications

• Active RN license in good standing.
• Experience coordinating MDS and RAI processes.
• Familiarity with CMS, OBRA and state regulations.

Responsibilities

• Maintain current MDS status for residents per OBRA and CMS guidelines.
• Schedule OBRA, Medicare PPS and Medicaid assessments.
• Coordinate and develop interdisciplinary plan of care.
• Monitor MDS and care plan documentation for accuracy and timeliness.
• Ensure compliance with regulations and electronic submissions.

MDS Coordinator Summary Description:

The MDS Coordinator under the direction of the Administrator or Director of Nursing, is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Payment System (PPS)/Medicaid Case mix Meeting. These duties are to be performed in a timely manner consistent with Centers for Medicare & Medicaid (CMS) guidelines and policies and procedures. This individual also establishes and maintains positive relationships with all members of the healthcare team.

Essential Functions and Responsibilities:

1. Maintains current MDS status of each patient/resident, in accordance with all state and federal guidelines, including the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable.
2. Schedules patients/residents for appropriate OBRA, Medicare PPS and/or Medicaid required assessments.
3. Communicates schedule to all other departments and families, as required.
4. Monitors clinical systems for changes in patient/resident condition.
5. Responds to patient/resident change in condition by coordinating patient/resident re-assessment and re-evaluation plan of care.
6. Strategically sets assessment reference date (ARD), in conjunction with the interdisciplinary team, to capture needs, care and services delivered to most accurately reflect the status of the patient/resident.
7. Accurately completes portions of MDS and CAAs as assigned, in accordance.with policies, procedures, and guidelines.
8. Assures compliance with federal, state, Medicare and Medicaid regulations, and facility policies and procedures as related to completion of an accurate electronic submission in a timely manner and coordination of the RAI process.
9. Coordinates and develops plan of care as required.
10. Monitors MDS and care plan documentation on all patients/residents, assuring that complete, orderly and chronological documentation is maintained in the file.
11. Maintains Medicare Certifications /Re-Certifications
12. Participates in QAPI /QM process
13. Maintains proficiency in software programs for MDS completion and is able to access and utilize appropriate reports.
