Job summary

Scott Lake Health and Rehabilitation is seeking an MDS Coordinator Nurse to manage the RAI/MDS process under the guidance of the Administrator or Director of Nursing. You will lead the daily PPS/Case mix meetings and ensure CMS regulatory compliance while collaborating with the healthcare team.

Responsibilities include maintaining MDS status, scheduling required assessments, coordinating with OBRA, PPS and Medicaid, and ensuring accurate documentation.

Qualifications

• Experience with MDS/RAI processes and CMS guidelines.
• Ability to coordinate with CMS, OBRA, and PPS processes.
• Strong organizational skills for ARD scheduling and care plans.

Responsibilities

• Maintains current MDS status for each patient in accordance with OBRA and CMS.
• Schedules OBRA, Medicare PPS and Medicaid assessments as required.
• Communicates schedule to departments and families as needed.
• Monitors clinical systems for changes in patient condition.
• Coordinates re-assessment and plan of care when condition changes.
• Sets ARD dates with the interdisciplinary team to reflect care needs.
• Completes MDS and CAAs accurately per policies and guidelines.
• Ensures compliance with federal and state regulations for electronic MDS submission.
• Develops and coordinates the care plan with the team.
• Maintains MDS and care plan documentation in patient files.
• Maintains Medicare Certifications / Re-Certifications.
• Participates in QAPI/QM process.
• Uses software to access and report on MDS activity.

MDS Coordinator Summary Description:

The MDS Coordinator under the direction of the Administrator or Director of Nursing, is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Payment System (PPS)/Medicaid Case mix Meeting. These duties are to be performed in a timely manner consistent with Centers for Medicare & Medicaid (CMS) guidelines and policies and procedures. This individual also establishes and maintains positive relationships with all members of the healthcare team.

Essential Functions and Responsibilities:

1. Maintains current MDS status of each patient/resident, in accordance with all state and federal guidelines, including the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable.
2. Schedules patients/residents for appropriate OBRA, Medicare PPS and/or Medicaid required assessments.
3. Communicates schedule to all other departments and families, as required.
4. Monitors clinical systems for changes in patient/resident condition.
5. Responds to patient/resident change in condition by coordinating patient/resident re‑assessment and re‑evaluation plan of care.
6. Strategically sets assessment reference date (ARD), in conjunction with the interdisciplinary team, to capture needs, care and services delivered to most accurately reflect the status of the patient/resident.
7. Accurately completes portions of MDS and CAAs as assigned, in accordance with policies, procedures, and guidelines.
8. Assures compliance with federal, state, Medicare and Medicaid regulations, and facility policies and procedures as related to completion of an accurate electronic submission in a timely manner and coordination of the RAI process.
9. Coordinates and develops plan of care as required.
10. Monitors MDS and care plan documentation on all patients/residents, assuring that complete, orderly and chronological documentation is maintained in the file.
11. Maintains Medicare Certifications /Re‑Certifications
12. Participates in QAPI /QM process
13. Maintains proficiency in software programs for MDS completion and is able to access and utilize appropriate reports.
